Acceptance Rollover FormUse this form if you have been accepted for one semester and would like to "rollover" your acceptance to the next, immediate semester (Fall to Spring, Spring to Summer, Summer to Fall) within 2 years of original acceptance or last attendance. Application to Convert ResidencyUse this form to apply to convert residency for tuition purposes by the last day of the 100% refund period of the semester for which you are applying the convert your residency status (see academic calendar). Include all relevant documentation for the information you provided on the application to convert residency form.
Authorization to Release Medical Information to Another UH System SchoolUse this form to authorize the release of your medical records to another UH system school
Change of Home Institution FormUse this form if you are currently a student or have been accepted at another campus and would like to make Hawaiʻi Community College your "home institution"
Change of Name/Preferred Name Request FormUse this form if a name change occurred, you will need to submit appropriate documentation to your UH home institution ARO office

Health Forms
Health Clearance FormUse this form to take to your physician or health care facility to fill in your completed health clearances. This document must be signed or stamped by a U.S. licensed physician or health care facility.
Religious ExemptionUse this form if immunizations conflict with your bonafide religious tenets and practices.
Medical ExemptionUse this form if you are unable to take certain immunizations required for post-secondary education that will have contraindications. The form must be signed by a U.S. licensed physician.

Add/Drop FormUse this form to accompany appeals or outside of normal STAR registration timelines. If STAR registration is available, you will be instructed to register or drop via the STAR GPS system.

Credit/No Credit Option formThe Credit/No Credit option is maintained to encourage students to broaden their education by taking courses outside of major requirements without affecting their grade point average. Please see form regarding certain restrictions that apply.

Enrollment VerificationUse this form to request certification of your enrollment with HawaiiCC.  Standard processing time within 7 business days.
Students - National Student ClearinghouseUse this online format for an expedited way to receive current enrollment, dates of attendance, and degree or school certificate information from the National Student Clearinghouse (NSC). Please read the NSC information as some disclaimers may apply. Important note: There will be an NSC fee that will be assessed. Regarding the current term information, you can use this option about a month after the term has started. If the information is needed sooner use the paper enrollment verification form

Student Appeal for Policy/Procedural Exception FormThe Student Appeal for Policy/Procedural Exception Form is used to request consideration for an exception to established HawaiiCC policy and procedure in the event of extenuating circumstances that is beyond the student's control, such as serious illness or injury, a death in the family, involuntary job transfers or military service, and institutional error. A request for an exception to another campus' policy or procedure must be made to that campus. HawaiiCC cannot make exceptions for another campus. Please attach documentation of extenuating circumstances before submitting this form.
